Improving Diabetic Kidney Disease Screening in Veterans Affairs Primary Care Clinics

An interdisciplinary team was formed in December 2022 at the Veterans Affairs (VA) Northeast Ohio Healthcare System, a large, high-complexity medical facility with 13 primary care locations serving ~100,000 veterans. Our veteran population is mostly male (92%) and older age (57% >65 years of age), ethnically diverse, and includes individuals residing in both urban and rural communities. Each of our primary care locations is an individual facility in a geographically convenient area of Northeast Ohio except for the primary care clinic attached to the main medical facility. The locations serve populations of different sizes and have their own staff, clinicians, social workers, pharmacists, and nurses.
