This quality improvement (QI) project was conducted in the Department of Family Medicine at NorthShore University HealthSystem, an integrated health system in the Chicago, IL, area with 13 ambulatory family medicine primary care practices caring for ~5,000 adult patients with type 1 or type 2 diabetes.
